The numerator is counting each visit in which vital signs are recorded, while the denominator is counting only unique patients. If you see a patient more than once during the reporting period and record vital signs each time, that patient is being counted each time in the numerator, but not the the denominator.&lt;/P&gt;&lt;P&gt;Since the attestation system will not allow you to enter a numerator that is larger than its denominator, report the number returned for the denominator in the calculations as the numerator and denominator. This will give you a 100% percentage. Depending on your practice, the calculator probably isn't double counting too many patients. The measure requires that you send reminders to at least 20% of the members of this patient set.&lt;/P&gt;&lt;P&gt;The denominator for Menu Set Measure 4 is number of active patients in your database who are under the age of 5 and over the age of 65. The numerator includes only the members of that patient set for whom you have generated a recall. You numerator will appear very low for the first several months, but for each month that you run recalls, your numerator will grow.
