The numerator for core measure 8 (vital signs) is larger than the denominator

Abstract:

The CMS Meaningful Use Reporting may display a numerator that is greater than the denominator, which represents a percentage greater than 100. The numerator is counting each visit in which vital signs are recorded, while the denominator is counting only unique patients. If you see a patient more than once during the reporting period and record vital signs each time, that patient is being counted each time in the numerator, but not the the denominator.

Since the attestation system will not allow you to enter a numerator that is larger than its denominator, report the number returned for the denominator in the calculations as the numerator and denominator. This will give you a 100% percentage. Depending on your practice, the calculator probably isn't double counting too many patients. The threshold for completing the measure is only 50%, so it's pretty clear that you are exceeding that.

If you are audited, send this article to the auditor to explain the anomaly.

This is a known issue, and we are working to resolve it in a future version of OfficeMate/ExamWRITER.

We generate recalls every month, so why is the numerator for Menu Set Measure 4 low or at zero?

Abstract:

Menu Set Measure 4 measures your effectiveness in sending patient reminders to patients who are under the age of 5 and over the age of 65. The measure requires that you send reminders to at least 20% of the members of this patient set.

The denominator for Menu Set Measure 4 is number of active patients in your database who are under the age of 5 and over the age of 65. The numerator includes only the members of that patient set for whom you have generated a recall. You numerator will appear very low for the first several months, but for each month that you run recalls, your numerator will grow. Because the numerator starts off so low and grows over time, this can be a difficult measure to meet in a 90-day reporting period, but should be easily achieved when you report a full year.

To generate recalls go to the OfficeMate Administration main window, click Tasks, select Correspondence [MU], and select Recalls [MU]. Only recalls generated in the Process Recalls window are counted in the numerator. For complete recall information, refer to the "Recalling Patients" chapter of the OfficeMate Administration User's Guide, which can be found under the Help menu.

Using Internal Marketing or Third-Party Recall Systems
If you are generating recalls through Internal Marketing, 4PatientCare, Websystem3, Demandforce, or any other mechanism, those recalls will not count toward your numerator in Menu Measure 4. You may continue to generate recalls as you have been doing, but you will need to also generate recalls (you don't need to send them) in the Process Recalls window.

How can I determine the percentage of Medicaid encounters for each provider?

Abstract:
Eligible providers whose caseloads include at least 30% Medicaid patients are eligible to receive up to $64,000 over the course of five years. Eligible providers cannot obtain incentives from both Medicaid and Medicare, but eligible hospitals can. Ophthalmologists nationwide are eligible for Medicaid EHR incentive programs. Optometrists are becoming eligible to participate in Medicaid EHR incentive programs in a growing number of states, and should check with their state board of optometry for eligibility.

There is currently no report that breaks down encounters by insurance and by provider and enumerates the percentages for each. A ticket has been opened to add a report to facilitate this in an upcoming version.

In the meantime, there is a workaround. Perform the following steps to determine the percentage of Medicaid patients for each provider:

Abstract:
The following information from CMS will help you meet CMS quality reporting measure NQF 0038 - Childhood Immunization Status. This article lists all of the codes for the denominator, numerator, and exclusions metrics for NQF 0038. Note that you do not need to request copies of patients' immunizations records and send them to CMS when you are attesting to meaningful use.
NQF 0038: The percentage of children 2 years of age who had four diphtheria, tetanus and acellular pertussis (DTaP); three polio (IPV); one measles, mumps and rubella (MMR); two H influenza type B (HiB); three hepatitis B (Hep B), one chicken pox (VZV); four pneumococcal conjugate (PCV); two hepatitis A (Hep A); two or three rotavirus (RV); and two influenza (flu) vaccines by their second birthday. The measure calculates a rate for each vaccine and two separate combination rates.

If you are using ExamWRITER and Microsoft HealthVault and you receive an error that states, "Timely Access Error - An error has occurred with the HealthVault system," follow the instructions below to automatically add the security certificate:
On the server, log in to the Terminal Server or workstation experiencing the error message as the Administrator.
Navigate to the OfficeMate or OMATE32 folder and double-click CertificateInstaller.exe.
If you receive an error while trying to run the Certificate Installer, navigate to the Documents and Settings\All Users\Application Data\Microsoft\Crypto\RSA folder and modify the Administrator's permissions in the MachineKeys folder so that they are set to Full Control.
If, after completing the instructions above, you continue to receive this error, add the security certificate manually by following the instructions below:

Open Internet Explorer, click Tools, and select Internet Options.
Click the Content tab.

Occasionally, you may need to exclude an exam from your meaningful use reporting because the exam is not billable, or you can't be expected to maintain the same level of care due to the nature of the encounter. For example, triage management.

Use the Meaningful Use Attestation Calculator to determine if you have met all of the objectives and their associated measures for meaningful use prior to completing attestation. Note that the tool does not calculate clinical quality measures; rather, itf allows you to assess your readiness to successfully complete the attestation process. After entering your core and menu set measure meaningful use data, the calculator will display whether you have met the necessary criteria for these objectives. You can then print a copy of the measures that you have entered and whether they have passed or failed. The calculator will indicate in red those measures for which the input values did not meet the required thresholds and will mark them as "failed."
