The numerator is counting each visit in which vital signs are recorded, while the denominator is counting only unique patients. If you see a patient more than once during the reporting period and record vital signs each time, that patient is being counted each time in the numerator, but not the the denominator.&lt;/P&gt;&lt;P&gt;Since the attestation system will not allow you to enter a numerator that is larger than its denominator, report the number returned for the denominator in the calculations as the numerator and denominator. This will give you a 100% percentage. Depending on your practice, the calculator probably isn't double counting too many patients.
