If you are running OfficeMate 7.2 or higher, the ZIP+4 (nine-digit ZIP code) information is automatically included in the billing provider's address on the CMS 1500. However, if you are running OfficeMate 7.3 or below and you are creating a CMS 1500 form for a Medicare patient, the ZIP+4 information will not automtically populate on the form and you will have to manually add it to box 33; in order for the ZIP+4 information to automatically populate on the form you will need to upgrade to OfficeMate 7.4.
The CMS 1500 pulls the ZIP code information from the Business tab of the Business Names window. If your practice's ZIP+4 information is not recorded on the Business tab, refer to the related article below for information on updating your practice's address.
Note: The hyphen is required in the ZIP+4 format (e.g., 92618-2211).
