Q13513 - HOWTO: Setting Up OfficeMate for PQRS

Note: Follow the directions in this article to troubleshoot autocoding or to add a new level II code to OfficeMate/ExamWRITER.

Level II CPT codes or PQRS codes are required by some MIPS Quality measures. Traditionally, these codes were reported as $0.00 line items on the CMS 1500 claims for Medicare and Medicaid insurances. Increasingly, however, incentive programs are moving away from claims-based submissions and relying on electronic or registry submissions.

Some or all of the level II CPT codes may be included in your OfficeMate/ExamWRITER procedure codes list. The latest codes are included in new installations of OfficeMate/ExamWRITER, but are not updated when existing installations are upgraded. You must verify that each of the level II CPT codes is included in your database. Create new services in the Products window and select the appropriate new level II procedure codes from the CPT drop-down menu. You must complete these tasks in order for the new level II procedure codes to be autocoded in ExamWRITER and recorded on fee slips in OfficeMate.

Designate the Qualifying Diagnosis Codes
A diagnosis that's associated with a MIPS Quality measure may require additional attention at the time of billing to ensure the appropriate level II codes are documented. Follow these instructions to flag the procedures that are associated with relevant Quality measures:

  1. From the OfficeMate Administration main window, click Setup.
  2. Select Third Party Setup.
  3. Click the Diagnosis Codes tab.
  4. Select the diagnosis code from the list on the left.
  5. Select the PQRI (Voluntary) check box.
  6. Repeat steps 4 and 5 for each qualifying diagnosis code.
  7. Click OK when you're finished.

Designate the Level II Procedure Codes
The level II procedure codes are the codes that are reported fro credit. Follow the steps below to add or modify level II procedure codes:

  1. From the OfficeMate Administration main window, click Setup.
  2. Select Third Party Setup.
  3. Click the Procedure Codes tab.
  4. Select the procedure code from the list on the left.
  5. Select the Level II CPT Codes check box.
  6. Repeat steps 2 and 3 for each qualifying procedure code.
  7. Click OK when you are finished.
Create Services for Level II Procedure Codes
Before you can record level II procedure code on fee slips, you must associate the level II code with a service. When creating in the Products window, select the appropriate new level II procedure code from the CPT/HCPCS Code drop-down menu. For information on creating new services, refer to the "Maintaining Product & Service Information" chapter of the OfficeMate Administration User's Guide.

Enable Level II Code Reminders
When you use a PQRS diagnosis code on a fee slip, you should also include a Level II CPT code. If you would like OfficeMate to remind you to select the level II CPT code, perform the following steps:
  1. From the OfficeMate Administration main window, click Setup.
  2. Select Preferences.
  3. Click the Finance tab.
  4. Select the Yes radio button for Display reminder for level II codes.
  5. Click OK when you are finished.

Automate PQRS
To automatically apply PQRS codes on exams in ExamWRITER for patients who use a specific primary insurance carrier, follow the instructions below:

  1. From the Business Names/Insurance tab (OfficeMate v8 and below users) or Insurance Billing Initial Setup window (OfficeMate v9 and above users), select an insurance carrier.
  2. Select Auto populate PQRI coding (OfficeMate v8 and below users) or click the Preferences tab and select Automate PQRI (OfficeMate v9 and above users).
  3. Click OK or Save.
  4. Open the Patient Demographics window and click the Insurance tab.
  5. Assign the insurance carrier that you set up for PQRS automation in steps 1–3 above to the patient as the patient's primary insurance carrier.
  6. Click OK.
